York Radiology Consultants is committed to protecting the privacy and confidentiality of your personal health information (PHI) in accordance with Ontario's Personal Health Information Protection Act, 2004 (PHIPA) and all applicable privacy legislation.
In the course of providing diagnostic imaging services, we may collect the following types of personal health information: your name, date of birth, Ontario health card number, contact information (phone, email, address), physician referral information and requisitions, diagnostic images and reports, and any relevant medical history provided by you or your physician.
Through our website, we may collect: your name, email address, phone number, and any information you voluntarily submit through our contact form or requisition upload feature.
Your personal health information is used for the following purposes: to provide the diagnostic imaging services requested by your physician, to communicate results and reports to your referring physician, to contact you regarding appointment scheduling and preparation, to meet legal and regulatory obligations, and to maintain the quality and safety of our services.
We employ administrative, physical, and technical safeguards to protect your personal health information, including: secure electronic systems for image storage and report transmission, access controls restricting PHI to authorized staff only, encrypted data transmission for all web-based submissions, staff training on privacy obligations and information handling, and physical security measures at all clinic locations.
When you upload a requisition through our website, your file and contact information are transmitted securely and sent directly to the clinic location you select via email. We do not store uploaded files on our web server. The transmission is protected by TLS encryption. By submitting a requisition, you consent to being contacted by our staff to schedule your appointment.
We do not sell, trade, or otherwise transfer your personal health information to third parties. We may disclose your PHI in the following circumstances: to your referring physician for the purpose of your care, as required or permitted by law (e.g., public health reporting), with your express consent, or to comply with a court order or legal process.
Under PHIPA, you have the right to: access your personal health information held by us, request corrections to your information, withdraw consent for certain uses of your information (subject to legal requirements), and file a complaint with the Information and Privacy Commissioner of Ontario if you believe your privacy rights have been violated.
Our website may use analytics tools to understand how visitors use the site. These tools may collect anonymized information such as browser type, pages visited, and time spent on the site. No personally identifiable health information is collected through analytics.
